Leslie, responding to your last point.... We have integration workshops right after close where each function creates their own plan but then we come back as a group to walk through together. Ongoing, we require our programs to have weekly cross functional integration management office meetings where everyone discusses current progress, issues and upcoming deliverables. There "should" be no excuse for surprises. Change management is monitoring over all but only as good as the representation at the table. Having a dedicated Integration Executive from the business who's obligation it is to lead the integration of the acquisition has made a huge impact in the success and quality of integrations too.

Here are a few that immediately come to mind based on my experience:
- Change fatigue
- Lack of adequate communication
- Leader/manager misalignment
- Culture integration (it's rarely well attended to)
- Lack of cross-function collaboration causing missed activities/deadlines and hard feelings.
